The rapid advancement of digital technology has accelerated the integration of Healthcare Information Systems (HIS), particularly Electronic Medical Records (EMR), across healthcare institutions worldwide. However, adoption among healthcare employees in developing countries, including Malaysia, remains uneven despite government-driven digitalisation initiatives under the Twelfth Malaysia Plan (RMK-12). This study reviews the adoption of the Value-Based Adoption Model (VAM) in the context of HIS research, with a focus on how perceived benefits (perceived usefulness and perceived enjoyment) and perceived sacrifices (perceived technicality and perceived fee) collectively shape employees' behavioural intention to use HIS. A review of recent literature confirms that VAM provides a robust theoretical foundation for understanding the cost-benefit evaluations that healthcare employees undertake when deciding whether to adopt new digital systems. The model has been successfully applied across various healthcare technology contexts, including internet healthcare platforms, mobile medical services, and AI-integrated clinical tools. Grounded in VAM, future researchers are encouraged to apply this framework in examining the predictors of HIS adoption across various healthcare settings and country contexts.
